At Mt. Horeb, we believe the movement of God in the world beckons us to participate! Thank you for your willingness to answer the call and join a team.

You can find basic details about each upcoming trip, including dates, cost, deadlines and Team Leader contact information, at mthorebchurch.org/missions. If you have questions about any portion of this application, please get in touch with meghan.sanders@mthorebchurch.org.

To be considered for any missional team, whether local or global, individuals must complete the following application and submit a non-refundable deposit, which will be applied to their overall trip cost. If the participant is under the age of 18, a legal guardian must also review and sign.

Please fill out this application completely. The amount of the non-refundable deposit will be listed when you select the trip below.

The Team Leader will notify the applicant once they have been accepted to the team.

NOTE: At the end of this application, you will find the Release and Waiver of Liability and the Assumption of Risk and Release of Liability. If you would like a physical copy of these forms, please request them from the Team Leader.

Hurricane Helene Relief

In late September 2024, Hurricane Helene caused widespread catastrophic damage in Western North Carolina. Through a partnership with On Mission Network, we will come alongside to help rebuild, restore, and repair the homes of those affected by Helene who are either uninsured or underinsured. The damage is so extensive that On Mission estimates it will be working in Western NC until 2034.

On Mission is currently in its first rebuild phase and needs skilled and unskilled volunteers to help people return to their homes in a safe, sanitary and functional way. There will be light to heavy construction, with something to do for everyone, regardless of skill level.

New Life Centers

New Life Centers (NLC) connects inner-city Chicago youth and their families to Christ and community through life-on-life relationships. Through mentoring, community care, education, sports, peacekeeping, and violence prevention, NLC comes alongside people in under-resourced communities to bring hope and foster long-term transformation.

Through a partnership with Mt. Horeb, New Life Centers expanded its food distribution (which feeds over 2000 people per week), migrant crisis shelter (shelters 240 people), mentoring and family support initiatives. While in Chicago, you will get to witness Mt. Horeb’s partnership with NLC and participate in some of their ministries, such as the food bank, community outreach programs and community events.

Appalatian Mountains

Red Bird Mission and Clinic have been providing ministries in the Appalachian Mountains since 1921. Red Bird serves an isolated, rural, distressed area, handicapped by chronic poverty, lack of jobs, poor housing and obstacles due to the rugged mountain terrain. Red Bird Mission and Clinic strive to meet the needs of their community through education, health and wellness, community outreach, economic opportunity and community housing improvement.

Mt. Horeb will be coming alongside Red Bird Mission in a variety of ways, the most prominent by serving in their work camp, which will include light to heavy construction. We will also be helping in the community thrift store and serving in the Senior Center.

If I were to be approved as a team member, I am also giving consent for video/photos/audio that include my name, likeness, or voice taken by representatives of Mt. Horeb Church to be used for promotional purposes within the church, on printed and electronic media, and on electronic platforms, including but not limited to Mt. Horeb's website, official social media accounts, and digital media. In addition, I am agreeing to treatment by medical staff if an emergency arises while on this trip.

Release and Waiver of Liability

This is a copy of the Release and Waiver of Liability required to travel with Mt. Horeb Church. You may also be asked to sign a physical copy of this form prior to departure. By typing my name below, I recognize that there are risks involved in participating in the mission trip and hereby assume all risk of injury, harm, damage, or death in connection with my participation in it. I hereby acknowledge that by engaging in this mission, I am subjecting myself to certain risks voluntarily, including and in addition to those risks which I normally face in my personal and business life, including but not limited to such things as health hazards due to poor food and water, diseases, pests, and poor sanitation; potential danger from lack of control over local population; potential injury while working; and inadequate medical facilities, etc. I understand and agree that neither Mt. Horeb Church nor its trustees, officers, directors, employees, agents or representatives may be held liable in any way for any injury, harm, damage, or death that may occur to me as a result of my participation in this mission trip and hereby release Mt. Horeb Church, its trustees, officers, directors, employees, agents and representatives from any injury, harm, damage or death, which may occur while I am participating in the mission trip. To the fullest extent permitted by law, I agree to save and hold harmless Mt. Horeb Church, its trustees, officers, directors, employees, agents and representatives from any claim by myself, my estate, heirs, successors, assigns or other persons arising out of my participation in the mission trip. Further, I hereby release and discharge the mission organizations which assisted in these arrangements, their agents, employees, and officers, from all claims, demands, actions, judgments, and executions which I ever had, or now have, or may have or claim to have, against the mission organizations, their agents, employees, and officers, and their successors or assigns for all personal injuries to personal property, real or personal, caused by, or arising out of, the above described mission service. If I am unable to do so, I authorize Mt. Horeb Church through its trustees, officers, directors, employees, agents or representatives to consent to any necessary examination, anesthetic, medical diagnosis, surgery, or treatment and/or hospital care rendered to me under the general or special supervision and on the advice of any physician and surgeon licensed to practice medicine by the state or country in which they practice, during the duration of the trip.

Passport

If you are applying for an international trip, you'll need to have a valid passport before airline tickets are purchased. Your passport must have six-month validity from the last date of the trip. For example, if the team returns on January 31, your passport must have an expiration date that is no sooner than July 31 of that year.
