At Mt. Horeb, we believe the movement of God in the world beckons us to participate! Thank you for your willingness to answer the call and join a team.

You can find basic details about each upcoming trip, including dates, cost, deadlines, and Team Leader contact information, at mthorebchurch.org/missions. If you have questions about any portion of this application, please contact the Team Leader.

To be considered for any missional team, local or global, individuals must fill out the following application and submit a non-refundable deposit. If the participant is under the age of 18, a legal guardian will have to review and sign as well.

Please fill out this application completely. The amount of the non-refundable deposit will be listed when you select the trip below.

The Team Leader will notify the applicant once they have been accepted to the team.

NOTE: At the end of this application you will find the Release and Waiver of Liability and the Assumption of Risk and Release of Liability. If you would like a physical copy of these forms, please request them from the Team Leader.

This trip is for adults only and will be serving alongside one of our mission partners, New Life Centers. New Life Centers of Chicagoland works in several areas, leading the nation in addressing the needs of their community through Education, Sports, Peach Keeping, and Community Development. This team will be serving in the food pantry, warehouse, participate in NLC's sports ministry, and serving at community block parties each night.

Hurricane Helene Relief

In late September 2024, Hurricane Helene caused widespread catastrophic damage in Western North Carolina. Through a partnership with On Mission Network, we will be coming alongside to help rebuild, restore, and repair homes of those affected by the Helene who are either uninsured or underinsured. The damage is so vast that On Mission estimates that they will be working in Western NC until 2034.

On Mission is currently still in its first rebuild phase and in need of skilled and unskilled volunteers to help get people back into their homes in a safe, sanitary, and functional way. There will be light to heavy construction and something to do for everyone of all skill levels.

Mangalagiri & Pune, India & Nepal

Grace Community (Prasanth Bethu) led by our former Pastoral Intern, Prasanth’s church serves in a community where Christians are less than 2% of the population and Christian persecution is pervasive. On this trip, we will participate in Grace Community’s community outreach programs.

Childhood Restoration Ministries (Sandeep Dongardive) rescues children at risk of sexual exploitation (Hope of Glory Children’s Home) and evangelizes and plants churches in the largest slum in their city. Hope of Glory houses, educates, and creates a family for the children in their care. Through Childhood Restoration Ministries' slum ministry, fifteen house churches have been planted with approximately 1000 people attending. They provide weekly services, Bible studies, a weekly children’s service, new believer training, and periodic community celebrations. On this trip, we will participate in their current ministries and spend time with the Hope of Glory children.

Frontier Missions (Birbal Tamang) plants churches and builds homes for the homeless (Homes of Hope) in small villages amongst the unreached Himalayan people of Nepal. Frontier Missions currently supports 14 communities with churches, community outreach programs, and home building. On this trip, we will visit Frontier’s churches and participate in community outreach.

If I were to be approved as a team member, I am also giving consent for video/photos/audio that include my name, likeness, or voice taken by representatives of Mt. Horeb Church to be used for promotional purposes within the church, on printed and electronic media, and on electronic platforms, including but not limited to Mt. Horeb's website, official social media accounts, and digital media. In addition, I am agreeing to treatment by medical staff if an emergency arises while on this trip.

Release and Waiver of Liability

This is a copy of the Release and Waiver of Liability required to travel with Mt. Horeb Church. You may also be asked to sign a physical copy of this form prior to departure. By typing my name below, I recognize that there are risks involved in participating in the mission trip and hereby assume all risk of injury, harm, damage, or death in connection with my participation in it. I hereby acknowledge that by engaging in this mission, I am subjecting myself to certain risks voluntarily, including and in addition to those risks which I normally face in my personal and business life, including but not limited to such things as health hazards due to poor food and water, diseases, pests, and poor sanitation; potential danger from lack of control over local population; potential injury while working; and inadequate medical facilities, etc. I understand and agree that neither Mt. Horeb Church nor its trustees, officers, directors, employees, agents or representatives may be held liable in any way for any injury, harm, damage, or death that may occur to me as a result of my participation in this mission trip and hereby release Mt. Horeb Church, its trustees, officers, directors, employees, agents and representatives from any injury, harm, damage or death, which may occur while I am participating in the mission trip. To the fullest extent permitted by law, I agree to save and hold harmless Mt. Horeb Church, its trustees, officers, directors, employees, agents and representatives from any claim by myself, my estate, heirs, successors, assigns or other persons arising out of my participation in the mission trip. Further, I hereby release and discharge the mission organizations which assisted in these arrangements, their agents, employees, and officers, from all claims, demands, actions, judgments, and executions which I ever had, or now have, or may have or claim to have, against the mission organizations, their agents, employees, and officers, and their successors or assigns for all personal injuries to personal property, real or personal, caused by, or arising out of, the above described mission service. If I am unable to do so, I authorize Mt. Horeb Church through its trustees, officers, directors, employees, agents or representatives to consent to any necessary examination, anesthetic, medical diagnosis, surgery, or treatment and/or hospital care rendered to me under the general or special supervision and on the advice of any physician and surgeon licensed to practice medicine by the state or country in which they practice, during the duration of the trip.

Passport

If you are applying for an international trip, you'll need to have a valid passport before airline tickets are purchased. Your passport must have six-month validity from the last date of the trip. For example, if the team returns on January 31, your passport must have an expiration date that is no sooner than July 31 of that year.
